The SECUR-IDTM “AYS” Personal Property Kit includes the following:
· AYS Bag – 30” x 66” with 3 possible size settings (15/20/25 cubic feet) for all of the individual’s belongings
· AYS Red Plastic Pull-Tie – pre-printed, unique, ID number; secures AYS bag at the proper size
· AYS SECUR-IDTM Personal Property Bag - same pre-printed, unique, ID number; wrist receipt stays with individual; rest of bag stored remotely to safely connect individual to their property
· AYS 4-part Form – same pre-printed, unique, ID number; used to provide details of individual & belongings; copies of form go: (1) on outside of AYS bag; (2) inside AYS bag; (3) inside AYS SECUR-ID personal property bag.
